Where this project started

This clinic group runs nine locations and had been limping along on a video vendor they'd adopted in a hurry during the pandemic. The vendor announced end-of-life for the product, which forced a decision the group had been putting off: build something that actually talked to their EHR, or bolt on another disconnected tool and keep asking clinicians to copy notes between two systems by hand.

They chose to build. Their one non-negotiable was that a video consultation had to land in the patient record automatically, with the same access rules and audit trail as an in-person visit.

How It Went

The work, including the part that went sideways

A team of six started in January 2025 with a five-month runway to a two-clinic pilot. The React Native app and the Next.js provider portal came together on schedule. The EHR integration didn't.

Three weeks before the pilot, we found that the EHR vendor's FHIR sandbox didn't behave like production: real endpoints enforced rate limits the sandbox never mentioned and omitted two fields our sync logic depended on. Our original design wrote to the record in real time during the consultation, and that design was now dead. We rebuilt the sync as a queued, retry-safe pipeline that reconciles the record within a minute of the session ending, and we pushed the pilot back nine days to test it properly rather than hitting the date with something fragile.

The pilot clinics went live in June, the remaining seven followed over the autumn, and the audit log has since sailed through the group's annual data protection review without a finding.
